The Nurse Liaison's role is multifaceted. They are the vital link between the potential patient and the rehabilitation facility. In this capacity, the transition between an acute hospitalization and rehabilitation is made as smooth as possible for the patient. The Nurse Liaison explains to the potential patient and his/her family members what to expect during the rehabilitation stay.
This explanation is usually given during the assessment of the potential patient. The Nurse Liaison performs the assessment at the request of the attending physician. This information, once obtained, is then submitted to the PNRC Physiatrist and the Admissions staff for review. If admitted, the patient will have met both HCFA's and Patricia Neal Rehabilitation Center's admission criteria. Tours of the Rehab Center are available.
Program Components
The Nurse Liaison role includes the following:
- Medical assessment of potential patients.
- Communicating the PNRC rehabilitation philosophy and continuum of services to potential patients, family members, and medical professionals.
- Collaborating with PNRC Case Management to assure quality care from the pre-admission process to discharge of the patient.
The Nurse Liaison is an RN/LPN currently licensed in the state of Tennessee. He/she has three or more years of diverse clinical experience, sound clinical judgment, and excellent assessment skills.
Referral Information
The Nurse Liaison is in constant communication with the Admissions Coordinator and is available to initiate assessment and pre-certification processes for potential rehabilitation candidates within one business day of referral.
